Hospital and Skilled Nursing Facility discharge follow up visits
Point of Care testing – blood sugar, urinalysis
Expanded services:
Same-day/next-day care for acute illnesses for established members.
Direct access to your doctor through office or telehealth visits, phone, email, and text (limited basis).
Care coordination with specialist (i.e. consultants, physical/occupational therapy, hospitalists).
After-hour, in-person acute appointments available on a limited basis at the discretion of the physician if medically indicated.
Worksite and home visits for an additional fee based upon the availability of the physician.
Referrals:
Dr. Castor will assist with referrals to specialists and help coordinate your care.
The office will also work with your insurance if prior authorization is required.
Services Not Covered Under Membership
Specialist visits, surgery or procedures performed outside of the office.
Urgent Care/Emergency Care – including UC/ER visits, physician services and transportation (ambulance).
Hospital services and coverage – we will gladly speak with hospitalists/specialists involved in your care during your in-patient. stay and arrange for hospital follow up care at the time of your discharge.
Nursing home or skilled nursing facility coverage. We will arrange for follow up care at the time of your discharge.
Insurance billing or filing on behalf of the member. Member will be solely responsible for this.
Long-term narcotic or controlled substance prescribing.
