Once your enrollment request is received and accepted, you will be charged the one-time registration fee and accepted as Dr. Castor’s patient.
$100 per individual/$150 per household (if household members register at the same time), one-time non-refundable fee, due at the time the enrollment request is accepted.
For worksite or home visits beyond 10 miles from Integra Direct Primary Care office will be charged at the IRS allowed mileage rate.
Dr. Castor will be able to care for you whether you are local or away from home through a variety of appointment options. Visits are by appointment only. There are no walk-in appointments.
Dr. Castor will roll out member enrollment to ensure that she is able to maintain the capacity of seeing new patients in a timely basis. Dr. Castor is currently accepting new memberships.
Once membership has reached capacity, her practice will be closed to new members until new membership slots open. Individuals who are interested in becoming a member at that time, can add their names to the waitlist. As openings become available, new members will be accepted from the wait list.
Members will continue to have their Medicare benefits for referrals to specialists, urgent/emergency room care, hospitalizations or outside testing/procedures. Dr. Castor will assist with any prior authorizations that are needed and will provide appropriate CPT codes when necessary.
