What’s included?

  • In office, telehealth, or phone visits for annual, follow-up and acute visits.
  • Every appointment scheduled with your physician.
  • Same day/next day care for acute illnesses for established members.
  • Access after-hours for urgent-care issues. (Routine-care issues will be addressed during office hours only, or the following business day.)
  • Direct access to your physician through phone, email, and text (limited basis).
  • No copayments or visit fees, except for point-of-care testing/services if performed.
  • Heavily discounted direct lab testing and imaging prices.
  • Fully transparent pricing so you know up front what the cost will be.
  • Referrals to specialists and testing. Your physician will work with your insurance if prior authorization is required.

Enrollment Request

Once your enrollment request is received and accepted, you will be charged the one-time registration fee and accepted as Dr. Castor’s patient.

Registration Fee:

$100 per individual/$150 per household (if household members register at the same time), one-time non-refundable fee, due at the time the enrollment request is accepted.

Membership Fee

There is a minimum commitment for three full months of service.
$100 – 18 to 39 years
$125 – 40 years and over
$300/semester – College Student

Additional Fees
(paid at time of visit)

  • Point of care testing
  • PAP smear/cytology
  • Skin lesion destruction
  • Durable medical equipment
  • Basic wound care
    (depending upon complexity)
  • Worksite or Home Visits

  • $100 per 30-minute visit during office hours.
  • $150 per 30-minute visit after office hours based upon availability of the physician.
  • Acute visits over 30 minutes will be pro-rated.
  • Hospice Care patients’ visits will not be charged and are included in the membership fee.
  • Other Fees


    For worksite or home visits beyond 10 miles from Integra Direct Primary Care office will be charged at the IRS allowed mileage rate.

    Non-Member Fees (offered upon availability of the physician)

    • $200 for up to 30 minutes.
    • $75 for every additional 15-minute increment.
    • Payment in full due at time of service.


    Dr. Castor will be able to care for you whether you are local or away from home through a variety of appointment options. Visits are by appointment only. There are no walk-in appointments.

    Office visits

    • Physical exams (up to 60 mins)
    • Wellness visits (up to 60 mins)
    • Follow-up for chronic diseases (up to 30 mins)
    • Acute/urgent issues (up to 30 mins)

    Virtual visits

    • Follow-up for chronic diseases
    • Acute/urgent visits if deemed appropriate.

    Phone visit

    • Follow-up for chronic disease (ie: blood pressure management)
    • Acute/urgent issues if deemed appropriate

    Wait List

    Dr. Castor will roll out member enrollment to ensure that she is able to maintain the capacity of seeing new patients in a timely basis. Dr. Castor is currently accepting new memberships. 

    Once membership has reached capacity, her practice will be closed to new members until new membership slots open. Individuals who are interested in becoming a member at that time, can add their names to the waitlist.  As openings become available, new members will be accepted from the wait list.


    Members will continue to have their Medicare benefits for referrals to specialists, urgent/emergency room care, hospitalizations or outside testing/procedures.  Dr. Castor will assist with any prior authorizations that are needed and will provide appropriate CPT codes when necessary.
